Directions:

If you are coming up from Main Street to Carew Street, take a right at Mercy Hospital, and parking will be on the left.

If you are coming from Chestnut Street, take a right onto Carew St. Then take a right at Mercy Hospital, and parking will be on the left.

Once you’ve entered the building, the office is located on the third floor in suite 323.

Why do I need a referral?

Various insurers may require a primary care referral; otherwise, the visit may not be covered.

A neurologist may not be the proper specialist for you to see. Your primary care doctor usually needs to make this decision.
Since we are consultants, we will usually work directly with your primary care physician, who needs to know the visit and why it was initiated.

We take most insurances. Please call our office and/or your insurance providers for specific questions.

How will I know if the office is closed for inclement weather?

We typically try to call our patients the night before if we plan to close the office. You may call if you are not sure. We may also make an announcement through the local news if necessary.

How do I get a copy of my medical records?

You must call the records department to obtain a release form which must be filled out in its entirety. This form can be picked up directly at the office or mailed to you.

There is typically a $10.00 processing fee which is waived if sending directly to another physician.

Turnaround times may be up to 72 hours with records prior to 2012 sometimes taking up to a week

How do I find my test results?

Usually, the doctor or his staff will notify you of abnormal results. Normal results are usually discussed at your next visit.
